Audition results were sent to directors by email on Saturday, November 30, 2024. If you have questions regarding your school's scores or have not received them by the date above, please contact your District Governor directly.
Before beginning the form, please scan your students’ completed Acceptance & Code of Conduct forms into ONE PDF.
Remember that student forms must have four signatures each to be complete: student, parent/guardian, teacher, and school administrator.

You will also need to print out the CMEA All State Chaperone Designation Form and have your school principal sign to ensure that a designated chaperone is attending with students.
Only complete the Director’s Acceptance Form once per school.
Do not mail your original student forms. Keep them for your records.
