Step 1: Give a copy of the following to EACH of your accepted CMASC students:
1. Student Acceptance Instructions & General Information
2. Student Acceptance & Code of Conduct Form (to be completed and returned to you, their director)
3. Link to order tickets, recordings, and merchandise.
Step 2: Complete the Director's Acceptance Form no later than Friday, December 13.
Step 3: Submit acceptance payments online or by check no later than Friday, December 13.

- Before beginning the form, please scan your students’ completed Acceptance & Code of Conduct forms into ONE PDF.
- Remember that student forms must have four signatures each to be complete: student, parent/guardian, teacher, and school administrator.
- Save your combined PDF with the filename School Name - Acceptance Forms.pdf
- You will also need to print out the CMEA All State Chaperone Designation Form and have your school principal sign to ensure that a designated chaperone is attending with students.
- Only complete the Director’s Acceptance Form once per school.
- Do not mail your original student forms. Keep them for your records.
